As part of our community's efforts to study the use of alcohol, drugs, and measure levels of depression and other risky behaviors, we ask that you participate in the Annual Healthy Community Survey. The survey is a tool implemented by the communities of the Community Welfare and Prevention Initiative (CPWI) . This survey is conducted annually until the end of autumn. The results are used as part of the needs assessment of the CPWI Community Coalition. The Behavior and Recovery Health Division sends specific community surveys to the Community Coalition Coordinators. Beneficiaries of the Drug Free Communities (DFC) are also invited to administer the Community Survey. 

The survey asks about their views on substance use among youth in the community. We will use this information to help us understand your thoughts and feelings to better serve the youth of our community. This survey is anonymous. We will not know who you are or what answers are yours. Your answers will be kept confidential and will be reported only together (combined with other responses). This survey is voluntary. That means you can skip any question or stop the survey at any time. The survey will take about 15 minutes.
